Incoming Student-Athlete Instructions

Information regarding your healthcare must be submitted to the Athletic Training staff prior to your opportunity to tryout for any intercollegiate athletic team at West Virginia University.
Note: All walk-on student-athletes must have their own medical insurance coverage which will cover athletic injuries and be accepted by medical providers in the Morgantown, W.Va., area.
In addition, you must provide the athletic training staff with the following:
- Copy of your insurance card (front and back)
- Proof of physical within last six months (must include date and physician signature)
- If you are under the age of 18, these forms must be co-signed by your parents or guardians

- Letter for Incoming Student-Athlete
- Stimulant Medication Letter
- Physical Preliminary Health Survey
